We offer home health services services for people with complex co-occurring physical and behavioral health needs to help bolster today’s fully-integrated care environment.
Lucid Living works to assure that psychiatric populations make it from institutionalized settings to primary care offices that are in the best position to reduce demand in other parts of the healthcare system. Our care team of nurse practitioners, registered nurses and certified peer counselors work with our network to respectfully keep our neighbors out of the hospital and serve as a bridge to whole-person care (primary care and behavioral health) that will be provided in primary care settings.
